How to Use:

This scorecard is a practical tool for healthcare organizations to assess interoperability across IT, clinical, and ERP systems. Each area is rated on a 1–5 scale (1 = high risk / poorly developed, 5 = fully optimized) to highlight gaps in system integration and support planning and prioritization.

It can guide discussions, improve coordination across departments, and provide oversight into system data and transfers. Use it to track progress over time and ensure a consistent, structured approach to strengthening interoperability across your organization.

System Integration & Data Flow:
- Evaluates data exchange between systems, including integration, ingestion, quality, and master data management to ensure accuracy and accessibility.

Governance, Security & Accountability:
- Reviews security, data governance, and roles to ensure policies are enforced, accountability is clear, and risks are managed.
Alignment & Collaboration:
- Assesses how interoperability goals, partner engagement, and cross-departmental information sharing support coordinated care and efficient operations.
Standards & Infrastructure:
- Examines use of interoperability standards, regulatory compliance, and infrastructure to ensure scalable, reliable system performance.

System Integration and Data Flow

Data Integration involves combining data from multiple clinical, operational, and administrative systems into a unified view so information can be accessed, shared, and used consistently. Includes mapping, normalizing formats, and routing through interfaces or engines.

How fully integrated are ancillary systems with the main hospital EHR?

Data Ingestion involves receiving data from various sources, processing it for quality and accuracy, and integrating it into the system in a timely manner. May involve APIs, interfaces, sFTP, or ETL tools, either point-to-point or through a centralized engine.

How effectively are data transfers routed through a transfer tool or engine?

Data Quality ensures data is complete, correct, consistent, and up-to-date for clinical and operational use. Includes monitoring the timeliness of updates and reliability of transfer tools.

How efficiently do your transfer tools provide accurate and timely data?

Governance, Security & Accountability

Security Risk means identifying, evaluating, and prioritizing potential risks to hospital systems and patient data. Includes audit logging, access control, and safeguards for transfer tools or engines.

How effectively does your system support audits of user access and activity?

Data Governance & Strategy involves establishing policies, standards, and processes that define how data is managed, protected, and aligned with organizational objectives. Includes ownership structures and quality controls.

Defined Responsibilities is establishing clear ownership and accountability for systems, workflows, and data management. Ensures appropriate access, defined approvers, and structured escalation paths.

Use the total of your ratings across all 11 domains to understand your interoperability readiness:

11–22: Significant gaps exist—consider evaluating a new solution.

23–33: Some gaps present—review your current setup and plan improvements.

34–44: Strong performance—most systems and processes are working well; focus on optimization.

45–55: Excellent—interoperability is well established; continue monitoring and refining.
